 COVID-19 FACILITY CONCERN COMPLAINT FORM 
Pursuant to the Memorandum of Agreement between Brevard Public Schools and Brevard Federation of Teachers, a protocol for teachers to anonymously report concerns about the facility conditions at a BPS site is to be developed. While the best and most expeditious way to address concerns is to bring them to the attention of the immediate supervisor or principal at the site, this form may be used if the complainant wishes to remain anonymous. Directions for submittal of this complaint form: Send this form and a copy of any supporting documentation or additional information in one of the following ways: 1) print and send via courier in a sealed envelope marked confidential to Director of Professional Standards and Labor Relations at ESF, 2)mail to same at 2700 Judge Fran Jamieson Way, Viera, FL 32940, 3) click the “submit” button to deliver to PSLR.
