• For teachers and staff to initiate the process of requesting a facility for any school-related activities (e.g., club meetings, concerts, testing, field trips, or other special events), CLICK HERE to download an Activity Request Form.

    Submit the attached form to Dr. Zeiler (Assistant Principal of Facilities) so he can seek approval for the event, as well as arrange for custodial staffing, the setup of tables/chairs, or any other support or resources needed to ensure a successful event.

    Approved events will be placed on the CHS Master Calendar that is displayed on the school website.
