Athletic Clearance will be live starting April 15th, 2023.  This digital platform is taking the place of the previous Athletic Eligibility Packet used in past years.  All forms will be completed digitally, except for the physical (EL2) and the ECG form.  These two forms (which can be found below this link) will need to be completed and uploaded to the Athletic Clearance Platform. 

This is the updated FHSAA Physical Form (EL2) for the 2023-24 school year.  The old EL2 form will not be accepted starting April 15th, 2023.  Only page 4 of the EL2 needs to be submitted to the school.  The other pages should be retained by either the doctor's office, or by the parent/ guardian.   

The ECG form needs to be filled out and turned in every year by students in order to play sports.
