MS PE Waiver Form

All MS students must have PE on their schedule. In order to opt out of PE due to playing a school or community sport, the parent or guardian must complete the above MS PE Waiver Form and return it to Cocoa HS. Please be sure to indicate the name of the student's counselor by circling (8th grade = Woods, 7th grade =Tribble, ESE = Grant).
