All 6th Graders trying out must have 2 Teacher Eval Forms filled out. Incoming 8th Graders must have 7 filled out, 1 for each teacher they have this semester. 

All completed forms must be received prior to tryout week.  Missing paperwork will result in dancers not being able to participate in tryouts.
