Hoover Middle School Families,

We are excited to inform you that all athletic packets will now be completed and submitted online for the 2025-2026 school year! Hoover Middle School will be utilizing Aktivate.com, a modern software platform that makes managing athletic activities seamless and efficient. We will not be accepting any paper packets for the 2025-2026 school year.   All athletics eligibility forms must be submitted through this website before participation in any activities.   This includes open gym, try-outs and practices/games.  Please keep in mind, athletic physicals are only valid for 365 days from day of exam. 

To get started, please find attached directions on how to create your account on Aktivate.com

All athletic forms will need to be downloaded from this website.  Once forms are completed, you will upload the required documentation.  Digital signatures will be required on specific forms from parent and student athletes.  Students must VIEW and provide necessary documentation that all courses from NFHS have been taken. 

Once ALL the information that is required is submitted, packets will be verified for completeness.  It is required that packets are submitted at least one week prior to participation to allow for review.  Complete packets turned in less than one week in advance, HOOVER does NOT guarantee verification in time for participation.  Ackivate.com will provide notifications on missing material and/or approval.  Hoover does NOT provide this information to families.  It is recommended to continue to monitor your AKTIVATE account for notifications.  Your student will NOT be allowed to participate until final approval from ACKTIVATE.
