Welcome to Johnson Middle School Athletics!

For the 2025-2026 school year, we will be transitioning to a paperless athletic clearance program called Aktivate. 

ALL students that want to participate in any sports MUST get their athletic clearance through Aktivate. 

Furthermore, all sports physicals MUST be completed on the newest (2/25) revision of the EL2 forms. 

  1. Please follow the instructions on the Aktivate Guide Sheet to create an account. 

  2. Get a sports physical at any number of places that offer them.  It does not have to be with your primary care provider.  It MUST BE on the 2/25 revised version of the EL2 forms.

  3. Upload your physical to Aktivate and complete the registration for athletic clearance. 

  4. When Coach Langlie verifies the information that was uploaded, you will be notified, and your child is cleared for athletic participation, including tryouts.

EL2 Medical Form (for physician)

This form should be pre-printed and brought to the physician of your choosing. Note: the physical will be eligible for 365 days.
