Annual Physical & Health Assessment 

Wellness Incentive Period: 9/1/2025 to 8/31/2026

Deductible Discount effective 1/1/2027

To earn the reduced Wellness deductible, employees and their spouses enrolled for medical coverage must EACH complete BOTH activities -- an annual physical and a health assessment.
